Dog Transport and Walking waiver 2023

Wag & Wheels LLC Transportation Services Waiver and Liability Release/ Photo Release and Agreement of Terms and Policies Wag & Wheels LLC care for our customers and our canine clients and we look forward to working with them. We strive to provide the safest care possible while transporting your canine. I understand that animals can have unpredictable reactions to being walked, playing with other dog(s)/Cat(s)/Pet(s) and transport which can cause illness or injury including, but not exclusive to: stress, vomiting, anxiety, self-injury, injury during interaction with other canines, dog bites, cuts, scratches, bone fracture, tissue damage, joint problems, diarrhea etc. I hereby waive, release, and discharge Wag & Wheels LLC and its owners and employees, staff from any and all liabilities that result in injury to, or the loss of my dog(s)/Cat(s)/Pet(s), or any other property of mine which arises in any way out of the services provided by Wag & Wheels LLC. I hereby agree to defend, indemnify, and hold harmless Wag & Wheels LLC, its owners, agents, officers, employees/staff, or volunteers from and against any and all claims of any nature including costs, expenses, veterinary care, and attorneys’ fees, which arise in any way out of the transportation or dog walking provided by Wag & Wheels LLC. I understand that Wag & Wheels LLC will be providing walking and/or transportation services to my dog(s)/Cat(s)/Pet(s) and will execute reasonable care throughout the walking and transportation process. I understand and accept the known and unknown risk of injury that can occur to my dog(s)/Cat(s)/Pet(s) during transportation and walks even with reasonable safety measures in place. I permit and authorize Wag & Wheels LLC to transport/walk my dog(s)/Cat(s)/Pet(s) to the documented daycare or facility noted on the client portal/booking. Furthermore, I permit Wag & Wheels LLC to be returned to my home address and released to family or friend. I authorize pick up and drop off at any facility for which my booking was made for my dog(s). I accept the rules, regulations, and policies promulgated by Wag & Wheels LLC and will comply with such. I understand that Wag & Wheels will secure dog(s)/Cat(s)/Pet(s) with safety tethers while in the vehicle and will leash them when transporting outside of vehicles, however I understand that animals can be unpredictable and I will not hold Wag & Wheels responsible if my dog(s)/Cat(s)/Pet(s) flees despite the use of such measures. Wag & Wheels LLC, at any time, can refuse a dog(s)/Cat(s)/Pet(s) from transport and walking services if the dog(s)/Cat(s)/Pet(s) exhibits aggression, uncooperative behavior, High anxiety or fear, refusal to enter or exit a vehicle, any condition concerning that the dog(s)/Cat(s)/Pet(s) may not be fit for transport or walks, if there is any concern that transportation/walks would not be in the best interest of the dog(s)/Cat(s)/Pet(s) or other dog(s)/Cat(s)/Pet(s) in transit. I acknowledge and accept that Wag & Wheels LLC is not a veterinary service and will not transport sick or injured animals for emergency care. I understand that group walks with other dogs can result in injury due to rough play or manmade hazards. I acknowledge that Wag & Wheels LLC will leash my dog on all walks unless it is mutually agreed upon to remove leashes on nature walks between Wag & Wheels LLC and the owners. Please see our cancellation policy. There are no refunds for the purchase of packages unless a dog(s)/Cat(s)/Pet(s) is deemed unsafe  (shows aggression or hurt upon arrival etc)  for service by Wag & Wheels LLC. If service is not able to be completed, except for client cancellation and illness, credit for the service will be returned to the client for future use (account credit) Any account credits are only good for 60 days after the credit date. Wag & Wheels, LLC reserves the right to deny service or terminate service if job differs from the original consult (including, but not exclusive to raspatory illness, any other contagious illness, bone fractures, or any other injuries and/or illness that Wag & Wheels was not made aware of) or if the job poses safety issues or financial concerns and the cancelation policy will still apply. Wag & Wheels, LLC also reserves the right to deny service or terminate service if illnesses or injuries occur, or, if incompatibility or inappropriate or uncomfortable situations arise. If dangerous or destructive behaviors preclude Wag & Wheels, LLC from providing further care of dog(s)/Cat(s)/Pet(s) , client authorizes pet(s) to be placed in a kennel If needed, with all charges therefrom to be charged to client. Cancelations policy will apply to the above. I hereby grant permission to Wag & Wheels LLC to use photographs and/or video taken of my dog(s) during services, in publications, news releases, online, and in other communications related to the Wag & Wheels LLC. Wag & Wheels time clock starts when we arrive at the property.  Wag & Wheels provides one free meet and greet, additional meet and greet, Client visit or any additional visit are subject to a fee starting at $20. I understand that Wag & Wheels LLC has exclusive rights to the photos taken. I understand the Wag & Wheels LLC reserves the right to cancel any trips that extend outside of the current service area as defined on wagnwheelsnh.com website. I understand and agree with the policy regarding return of dog(s)/Cat(s)/Pet(s) if the client does not confirm a reservation at the facility that the dog(s)/Cat(s)/Pet(s) is to be transported to. As part of this policy, I understand that if the daycare cannot accommodate the dog(s)/Cat(s)/Pet(s) that they will be returned to the home address and full round trip ticket price will be charged along with a $50 service fee per dog. I understand and agree to confirm that daycare or appointments are booked with the destination facility and that payment for services rendered by the facility will be arranged beforehand by the client. This agreement shall be governed by, and construed in accordance with, the laws of the State of New Hampshire. I, the undersigned, affirm that I am at least 18 years of age and I freely sign this Waiver and Liability Release. I have read the Waiver and Liability Release and fully understand that by signing below, I am giving up legal rights and/or remedies which may otherwise be available to me regarding any losses I may sustain. I agree that if any portion is held invalid that the remainder will continue in full legal force and effect. If registering on-line, my online signature shall substitute for and have the same legal effect as an original form signature.

We walk rain or shine! Your pup’s walk will continue in most weather conditions. However, in cases of extreme weather—including heavy rain, thunderstorms, heave snow, or extreme cold/heat —we may shorten walks to ensure the safety and comfort of your pet.

For cancellations due to weather, please refer to our Cancellation Policy for details on timing and fees.

We cannot guarantee that you will have the same walker every time. While we will do our best to accommodate requests for consistent walkers, we cannot promise it.

We schedule visits within 2-hour time blocks. While we provide an estimated time of arrival (ETA) for your visit, the actual visit may not occur precisely at that time. However, rest assured your visit will be completed within the scheduled time block.

Cancellation policy

Cancellations can be made through the Precise Pet App or by calling or texting us, but must adhere to the following timeframes:

1. Weekday Dog Walking and Adventure walk Services:
– Cancellations must be received 48 hours before the scheduled service.

2. Transport
– to and from boarding, daycare and grooming local: Cancellations must be received 24 hours before the scheduled service.
– All other types of transportation: Cancellations must be received 72 hours before the scheduled service.

3. Weekend Pet Care Service: Cancellations must be received 72 hours before the scheduled service.

4. Vacation & Holiday Service: Cancellations must be received at least 7 days prior to the scheduled service.

5. Overnight Pet Sitting Service: 

– Cancellations must be received at least 14 days prior to the scheduled service to receive 25% credit back 

– Cancellations must be received at least 7 days prior to the scheduled service to not be charged the full amount of pet sitting request. 

** Please note that “change request” within 48 hrs is still a cancellation

For snow days ONLY,

we understand they can be hard to predict and might be called in the evening before, if you know you will not need our services, please let us know right away so that you will not be charged. On days of inclement weather, cancellations made after 8AM will be charged the full price of the service. We HIGHLY encourage you to cancel through the Precise Pet App/ website, as we will try to focus on driving when the roads are slippery.

If a cancellation request is made after the specified deadline, the client will be charged the full price of the scheduled service. Please note that these service charges are non-transferable and cannot be exchanged or redeemed for a different day, as our staff has been assigned to the scheduled visit already.

Cancellation requests can be submitted through the Precise Pet App, or you can reach us by phone or text. Please be aware that we do not accept cancellations via social media messages, as they are not monitored as frequently.

We appreciate your understanding and cooperation in helping us maintain the highest level of service for you and your pets.

Illness

Wag & Wheels, LLC reserves the right to deny service or terminate service if job differs from original consult (including, but not exclusive to raspatory illness, any other contagious illness, bone fractures or any other injures and/or illness that wag & Wheels was not made aware of) or if the job poses safety issues or financial concerns and the cancelation policy will still apply. Wag & Wheels, LLC also reserves the right to deny service or terminate service if illnesses or injuries occur, or, if incompatibility or inappropriate or uncomfortable situations arise. If dangerous or destructive behaviors preclude Wag & Wheels, LLC from providing further care of pet(s), client authorizes pet(s) to be placed in a kennel If needed, with all charges therefrom to be charged to client. Cancelations policy will apply to the above. 

Credit Card Agreement

Wag & Wheels requires a credit card on file before booking. Your credit card will NOT be charged until you request service. Services that are not part of a package will be charged up to 7 days before or on the day of service before the service is rendered. Packages that are purchased will be billed on or the next day of purchasing the package.

Our system will automatically generate and email you an invoice. If you notice any discrepancies, please email us right away so we can look into it for you. If you do not pay your invoice by noon, Wag & Wheels will charge your card between 12 PM and 6 PM.

All cards on file are subject to an additional 2% fee per transaction.

Refund policy

There are no refunds for the purchase of packages or paid invoices unless a dog is deemed unsafe for travel by Wag & Wheels LLC. If service is not able to be completed, except for client cancellation, credit for the trip will be returned to the client for future use (account credit) Any account credits are only good for 60 days after credit date.

Key Policy

Please note that we do not accept hard copies of keys. Instead, we can leave a secure lockbox at your property for your convenience. If you prefer, you can also purchase a lockbox yourself and provide us with the access code.

If you live in a secured building that requires a key, We recommend to seeing if you are allowed to put out a lockbox. If they do not allow you to please advise us of the next steps 

Dog Walking Package Policy

Expiration: Packages are valid for 90 days from the first visit. The first visit must occur within 30 days of purchase.

Unused Visits: Unused visits after 90 days are forfeited. Extensions are only granted for approved emergencies.

Refunds: Packages are non-refundable.

Transferability: Packages are non-transferable and apply only to the pet(s) listed at purchase.

Temporary Fuel Surcharge

Effective April 7, 2026 — Wag & Wheels LLC
 

$1.00 – $15.00 per dog walk — applied as a separate line item on all invoices. Boarding and other services are not affected at this time.

Why this surcharge exists:
Fuel costs are a meaningful part of operating a mobile pet care service. When pump prices rise significantly, they affect every member of our team. This surcharge helps us continue providing reliable, high-quality service without reducing coverage or walker availability.
How it appears on your invoice

The surcharge is always listed as a separate line item — it is never bundled into your service rate. This keeps your pricing transparent and makes it easy to see when the surcharge is added or removed.

Duration & review:
This surcharge is temporary. We review fuel costs on a quarterly basis. Our next scheduled review is July 2026. If prices return to normal levels, the surcharge will be removed and clients will be notified.

Pet sitting

1. Liability Policies Wag & Wheels, LLC, agrees to provide services stated in this contract in a reliable and trustworthy manner. In consideration of these services and as an express condition thereof, the client expressly waives any and all claims against Wag & Wheels, LLC unless arising from gross negligence on the part of Wag & Wheels, LLC. Client agrees to notify Wag & Wheels, LLC of any concerns within 24 hours of returning home. Returning home or visiting home while pet sitting: a. Wag & Wheels, LLC shall not be held responsible for pets that cause damage to furniture, flooring/woodwork, carpeting, walls etc. while the pet sitter is not present and/or when the pet returns home. b. Wag & Wheels, LLC shall not be held responsible for the loss, injury, death, or actions of any pet(s) the client has let outside or has instructed Wag & Wheels, LLC to allow outside while sitter is not there. *This includes pets with doggie doors and outdoor pets. c. The client understands that all pets (where appropriate) must have a veterinarian and must be up to date on the rabies vaccination. Proof of vaccination is required. Client agrees to reimburse Wag & Wheels, LLC Pet Care for all costs (including, but not limited to, medical care and lost wages) associated with contracting any ailments while exposed to pet(s). d. No fence system is 100 percent secure for your pet’s safety. Wag & Wheels, LLC does not accept responsibility or liability for any pets that escape, are injured, become lost or fatally injured when left outdoors or given access to fenced in areas. This included electronic, wood, metal and all other fence types. All pets with electronic fences must wear their collars with fresh batteries. Likewise, Wag & Wheels, LLC is not responsible or liable for pets that escape, are injured, become lost, or fatally injured when outdoors in areas that are NOT fenced in. Wag & Wheels, LLC is not responsible for critters or animals i.e. coyotes, snakes, etc. that enter client’s yard or jump client’s fence and injure or fatally attack client’s pet(s). e. Wag & Wheels, LLC Pet Care will not sit for acutely ill animals or those with uncontrolled medical conditions. We suggest that your pet board with a veterinarian. f. Wag & Wheels, LLC Pet Care does not accept aggressive animals. If pet(s) exhibits aggressive behavior that poses a risk to the pet sitter or prohibits them from caring for the pet(s), service will not be provided. Client agrees to be responsible for all costs (including, but not limited to, medical care, attorney fees, etc.) if client’s pet(s) should bite another person or animal. g. Due to pet(s) excitement to see us, “door darting” may be attempted and an escape could occur. We take every precaution to prevent this from occurring and require that all dogs and cats under our care wear a collar with an ID tag or collars with the pet’ name and owner’s phone number embroidered or indelibly marked onto the collar. h. Client authorizes Wag & Wheels, LLC to obtain the services of a locksmith should a key, lock pad, keyless entry electronic door pad, or garage door opener malfunction. Client is responsible for all charges. i. Client is responsible for making arrangements for snow removal. Visits may not be made in snow covered driveways and/or walkways because of safety reasons. j. Wag & Wheels, LLC will not be responsible for any keys the client has asked to be mailed. k. Job sharing is not covered under our insurance. We can NOT be responsible for your pet or home if another service provider, friend or family member enters your home while we are not there.

2. While your pet is staying with us, they may possibly be in proximity with, or exposed to other pets. A. You acknowledge and agree that in the unlikely event, your pet is injured by another pet, you will not hold us responsible for the injury. B. If your pet injures another pet, you will be solely responsible for any injury that occurs, whether to your pet or another. C. Communicable diseases: All pets staying with Wag & Wheels are required to be fully vaccinated. We recommend all vaccines and boosters are completed at least one week prior to your drop-off date in order for your pet to receive the full benefit of those vaccines. However, it is still possible for a pet to become ill, even when vaccinated, especially when stressed by being away from home. This is not due to any circumstances or conditions, and you agree that you will not hold us liable in the event your pet becomes ill during or after its stay.

3. Cancellation Policy A. Cancellations must be received at least 14 days prior to the scheduled service to receive 25% credit back B. Cancellations must be received at least 7 days prior to the scheduled service to not be charged the full amount of pet sitting request. 

4. Cancellation of Contract Wag & Wheels, LLC reserves the right to deny service or terminate service if job differs from original consult (including, but not exclusive to raspatory illness, any other contagious illness, bone fractures or any other injures and/or illness that wag & Wheels was not made aware of) or if the job poses safety issues or financial concerns and the cancelation policy will still apply. Wag & Wheels, LLC also reserves the right to deny service or terminate service if illnesses or injuries occur, or, if incompatibility or inappropriate or uncomfortable situations arise. If dangerous or destructive behaviors preclude Wag & Wheels, LLC from providing further care of pet(s), client authorizes pet(s) to be placed in a kennel, with all charges therefrom to be charged to client. Client releases Wag & Wheels, LLC from any and all liabilities related to transportation of pet(s). Client may terminate this agreement at any time in writing and agrees to pay immediately any outstanding service invoices. Client agrees to arrange for another qualified sitter to fulfill the responsibilities set forth in this contract.



5. Emergencies Client agrees to authorize Wag & Wheels, LLC to handle any emergencies that may arise. Wag & Wheels, LLC will make every effort to contact client, however client gives Wag & Wheels, LLC authority to act in the pet’s/home’s best interest. Client agrees to an hourly rate of $30 is for all medical emergencies. If pet(s) become ill while under the care of Wag & Wheels, LLC, client authorizes Wag & Wheels, LLC to transport the pet(s) to client’s veterinarian or to one who is available, including after-hour emergency clinics. Client agrees to reimburse Wag & Wheels, LLC for any medical expenses incurred within five days of returning home or a late charge of $20 will be applied. Client releases Wag & Wheels, LLC from any and all liabilities related to treatment and transportation resulting from emergencies. During a home emergency/ issue (utility outage, security issue, fire, damage, etc) Wag & Wheel will contact you RIGHT away. In the event you do not answer, we will contact your emergency contact. If needed, pet(s) may be relocated to a safe area (our home or a hotel) until the issue is resolved. Wag & Wheels, LLC requires you to have a secondary responsible party to take care of your pet(s) in the event of unforeseen circumstances beyond our control such as inclement weather, natural disasters, and acts of terrorism. Wag & Wheels, LLC is entrusted to use best judgment in caring for your pet(s) and home and will be held harmless for consequences related to such decisions. In the event of personal emergency or illness of Pet Sitter, client authorizes Wag & Wheels, LLC to arrange for another qualified person to fulfill responsibilities as set forth on this contract. Every attempt will be made to notify client regarding such situation.

6. Payment Arrangement Client acknowledges that payment is due immediately upon first day of scheduled service period in an agreed specified location. A 25% nonrefundable deposit is due upon accepting Quote or booking services and is applied towards final bill. In the event of additional unforeseen visits or other costs (such as food, supplies, vet fees, etc.), payment is expected within five days of the completion of services or a late charge of $20 will be applied

7. Updates Client is responsible for providing Wag & Wheels, LLC with updates or changes regarding care of your pet(s) health, feeding schedule, behavior, etc. 

8. Confirm Travel Plans Client agrees to Wag & Wheels, LLC three days prior to departure to confirm travel plans.

9. Video Surveillance Use of Video Surveillance If you will be using surveillance cameras in the home, please initial below. We require that cameras not be allowed in restrooms or rooms used for overnight services ( including comma area, they must be turned OFF). Wag & Wheels has the right to turn off or unplug any camera on the INSIDE if the home. You agree not to share any video of Wag & Wheels LLC staff on social media or any other venues without our knowledge of or approval.



10. Returning Home Early From a Trip Wag & Wheels, LLC does NOT provide refunds or future credits for pre-paid services rendered. No refund or credit for early return, unless in cases of extreme emergency (i.e. death in family, or major family illness). This policy is in effect to ensure that we adhere to our strict no overbooking policy, which means we may turn away other clients to hold a spot for your pet to ensure they receive the personal love and care they deserve.

11. Upon Returning Home A. If the pet is dropped off while the client is not present and instructed to return prior to the client’s return, the Client agrees to notify Wag & Wheels, LLC via text or phone upon the day/night of returning home from a trip to confirm they are home safely. 

12. Veterinarian release During my absence, wag & Wheels, LLC will be caring for my pet(s). In the event of an emergency, I authorize you (veterinarian) to administer medical treatment and will be responsible for payment to you (veterinarian) upon my return. I, Client ( see signature below), give Wag & Wheels, LLC permission to transport my pet(s) to the above veterinarian and authorize treatment in the event of an emergency or sickness. If this veterinarian is not available, I authorize Wag & Wheels, LLC to transport my pet(s) to a veterinarian of choice and authorize treatment. If emergency care is needed after regular office hours, my pet(s) may be taken to the nearest Veterinarian Emergency Clinic/Hospital. I give permission to Wag & Wheels, LLC to approve treatment up to $ SEE AMOUNT BELOW (input maximum dollar amount or “no limit”). I agree to be responsible for all charges upon my return including, but not limited to, vet fees, extra visit fees and transportation fees. I agree to authorize veterinarian to euthanize my pet in extreme circumstances after all reasonable attempts have been made to reach me or my emergency contact. I agree that Wag & Wheels, LLC is released from all liability related to transportation to and from veterinarian and treatment for sickness or emergency.This release will remain valid for 12 months of sign date.



PLEASE NOTE: · The utmost care will be given in watching your pet(s). However, due to the extreme unpredictability of animals we cannot accept responsibility for any mishaps of any extraordinary or unusual nature (i.e. animal bites, furniture damage, accidental death, etc.) or any complications in administering medications to the animal(s). Nor can we be liable for injury, disappearances, or death of pet(s) with access to the outdoors. · For all Dog walk & Transport waiver please refer to Pet Walking and Transport waiver(required) in your portal Client acknowledges that by signing below, he/she fully understands and agrees to the contents of this contract. 

*Any urgent pet sit less than 5-day notice is subject to a fee of up to $250 

Boarding

Dog Boarding Waiver:

Dog Boarding Waiver 1. Liability Policies Wag & Wheels, LLC agrees to provide boarding services in a reliable and trustworthy manner. As a condition of these services, the client expressly waives any claims against Wag & Wheels, LLC unless arising from gross negligence. The client agrees to notify Wag & Wheels, LLC of any concerns within 24 hours of picking up their pet. a. Wag & Wheels, LLC is not responsible for pets that cause harm to themselves, other pets, or property due to their behavior or pre-existing conditions. b. The client is required to provide proof of up-to-date vaccinations, including rabies, prior to boarding. Wag & Wheels, LLC will not be liable for any illnesses contracted due to incomplete or missing vaccinations. c. Wag & Wheels, LLC does not accept responsibility for pets that escape, are injured, or become lost while in secure outdoor or indoor environments unless due to gross negligence. d. Wag & Wheels, LLC reserves the right to refuse boarding for acutely ill or aggressive animals. If a pet becomes unmanageable or poses a risk, services will be terminated, and the pet may be relocated at the client’s expense. e. All pets must wear identification tags with the owner’s contact information during their stay. 2. Health and Safety** a. All pets must be free from communicable diseases and parasites before boarding. If any issues arise, the client is responsible for veterinary expenses. b. Pets may interact with other boarded animals. If your pet is injured or injures another pet, the client assumes full responsibility for any resulting veterinary or medical costs. c. Stress-related illnesses can occur in a boarding environment despite proper care. The client agrees not to hold Wag & Wheels, LLC liable for such occurrences. 3. Emergencies** In the event of an emergency: a. Wag & Wheels, LLC will attempt to contact the client or their emergency contact. If unavailable, Wag & Wheels, LLC is authorized to act in the pet’s best interest, including seeking veterinary care. b. Clients are responsible for all costs incurred during emergencies, including transportation and treatment fees. An emergency fee of $30/hour may apply. 4. Payment and Cancellation Policies** a. A non-refundable deposit of 25% is required upon booking. The remaining balance is due at drop-off. b. Cancellations must be made at least 14 days in advance to receive a partial credit. Cancellations made within 7 days will result in forfeiture of the full amount. c. No refunds are provided for early pick-ups or unused days. 5. Facility Use** a. Wag & Wheels, LLC ensures that all boarding areas are clean, safe, and secure. However, no facility can guarantee a 100% risk-free environment. b. Clients must provide all necessary supplies, including food, medications, and comfort items. Additional fees may apply if supplies are not provided. 6. Acknowledgment and Agreement** By signing below, the client acknowledges and agrees to the terms and conditions outlined in this waiver. This agreement is valid for 12 months from the sign date.
